Transportation Program
Chickaloon Village Traditional Council’s Health and Social Services Department offers transportation for medical, dental, vision, and behavioral health visits.
The following considerations apply to all transports:
- All appointments need to be scheduled between 10 a.m. and 2 p.m. to ensure there is enough time to pick you and other clients up.
- To be eligible for transports, you may be asked to complete the transportation enrollment forms, a copy of your Certificate of Degree of Indian Blood (CDIB), and/or other identification.
- There are limited outreach appointments available for Elders and low-income clients who need to pay bills, shop, and run other errands.
- Medical, dental, vision, and behavioral health transports take precedence over outreach appointments.
CVTC offers transportation for medical, dental, vision, and behavioral health visits to the C’eyiits’ Hwnax Life House Community Health Center. CVTC offers handicap access (upon request) and is available to all residents in our service area (MP 70 to MP 40) as dates, times, and seating are available.
Transportation Program Closures
CVTC is closed on all major holidays and closes when weather conditions are extreme; this is for your safety and the safety of the drivers. You will be notified of cancellations at our earliest convenience, and we will work with you to help reschedule any of your appointments.
Transportation drivers are not available on the first Wednesday of each month; therefore, there are no transports scheduled.
