Transportation Program

The Chickaloon Village Traditional Council offers transportation for medical, dental, vision, and behavioral health visits at Life House Clinic.

The following considerations apply to all transports:

  • All appointments need to be scheduled between 10 a.m. and 2 p.m. to ensure time to pick you and other clients up and have time to transport you safely to your destination.
  • To be eligible for transports, you may be asked to complete the transportation enrollment forms and to provide copied of your CIB and/or other identification.
  • Proof of IHS beneficiary status is not required for appointments at the Life House Clinic.
  • We have limited outreach appointments available for Elders and low-income clients who need to pay bills, shop, and run other errands.
  • Medical, dental, vision, and behavioral health transports take precedence over outreach appointments.

Transportation Program Closures

We are closed on all major holidays. We also close when weather conditions are extreme; this is for your safety and the safety of the drivers. You will be notified of cancellations at our earliest convenience, and we will work to help with rescheduling any of your appointments.

The second Wednesday of each month, our transportation drivers are required to attend a staff meeting; therefore, there are no transports scheduled on these days.
